The division is unglamorous. A 1,000 mg vial contains ten lots of 100 mg, so every price in the second column divides by ten to give the rate in the fifth. $52.00 becomes $5.20, $104.00 becomes $10.40, $93.60 becomes $9.36.

Read backwards, $1.00 buys 19.2 mg at the code rate and 9.6 mg at list. That is the same comparison inverted, and quoting both in the same breath is how price pages accidentally claim a discount twice.

How much does NAD+ cost per milligram, and why that unit misleads

Per milligram, NAD+ with the code costs $0.052. The number is arithmetically correct and practically useless, because the only figures a reader has to compare it against come from vials holding a hundredth as much material.

The last column is notional for the three peptides, because no 10 mg vial contains 100 mg of anything. It is there to show the size of the gap: NAD+ costs about a ninety-fifth of retatrutide per milligram and a forty-seventh of BPC-157, and none of those ratios tells you anything, because the quantities in play differ by roughly the same factor.

That is the whole argument for the unit change. A coenzyme handled in hundreds of milligrams and a receptor agonist handled in fractions of a milligram do not belong in the same column. The vial arrives as dry powder with no concentration at all. Whatever you choose as the diluent volume sets both the mg/mL and the smallest quantity a barrel can resolve, and it does not touch the price at all.

At 10 mL the arithmetic is as clean as it gets anywhere on this site. One unit on a U-100 barrel is 0.01 mL, which at 100 mg/mL holds exactly 1 mg, so 10 units draws 10 mg and a full 100 unit barrel holds 100 mg. The made-up vial is ten of those barrels by volume.

Note what the last column does, which is nothing. Cost per 100 mg is a property of the purchase, and cost per unit is a property of the dilution. Two people paying the same $52.00 can quote wildly different cost per unit figures and both be right, which is why cost per unit only compares across vials when the concentrations match.

NAD IV price: the structure, and the fraction of it that is material

NAD+ has something no research peptide on this site has, which is a real service channel. Drip bars, wellness clinics and med spas sell NAD+ infusions openly, and that is where most people first meet a price for it.

Those prices are structured, not listed. A session is billed as a session, sometimes stepped by the amount infused, usually discounted inside a package of several, occasionally folded into a membership. There is no national rate to look up, no formulary position and no entry in the FDA approved drug database to price against, because no NAD+ drug product has been approved. We are not going to quote a range for it either, because the ranges circulating online trace back to unsourced aggregation rather than to any published schedule.

What we can do is price the material, which is the half of the transaction that has an objective number attached.

Take whatever figure a clinic quotes you, find the row that matches what they say is going in the bag, and subtract. The remainder is the room, the chair, the line, the clinician, the sterile compounding and the margin. That is a legitimate thing to charge for and it is most of the price, which is the point worth understanding before comparing the two channels at all.

The best rung on that ladder is $9.36 per 100 mg, which is 80% more than the $5.20 a single vial costs with the code, and getting there means committing $936.00 and taking delivery of ten grams. A 10% discount cannot catch a 50% one.

A rate per 100 mg measures one thing. Four things it does not measure move the real cost of an NAD+ purchase more than any discount on this page.

  • Grade, and the endotoxin question underneath it. On 30 October 2024 FDA published a notice stating it "is aware of compounders using food-grade nicotinamide adenine dinucleotide (NAD+) sold by repackagers to make intravenous products", that food-grade ingredients "are not suitable for compounding sterile drugs without appropriate processing, due to the high risk of contamination with microbes and endotoxins", and that it had received adverse event reports after use of NAD+ injectable drugs including "severe chills, shaking, vomiting and fatigue" consistent with excessive endotoxin levels. A certificate that reports identity and purity by HPLC and mass spec has not answered that question. Look for a bacterial endotoxins result, the LAL assay, named on the batch certificate.
  • Batch traceability. Ascension publishes two third-party assays for the NAD+ 1,000 mg listing: Kovera Labs, batch 25-05260628, tested 14 June 2026, and MZ Biolabs, batch 25-01260229, tested 26 January 2026. Match the code on the certificate to the code printed on the vial in your hand before the price per 100 mg means anything, because the whole calculation rests on the labelled milligrams being real.
  • Regulatory position. Nicotinamide Adenine Dinucleotide (NAD) sits in FDA category 1, bulk drug substances under evaluation, on the 503A category list updated 14 May 2026. Category 1 means FDA does not intend to take action against a compounder using the substance provided the conditions in the guidance are met.   • Landed cost. A flat shipping charge divides by the ten lots of 100 mg in the vial rather than scaling with them. A $12.00 charge on a single vial adds $1.20 per 100 mg, which is 23% on top of the $5.20 you thought you were paying. Spread across five vials that clear the free carriage threshold it adds nothing at all.

One smaller effect belongs here for completeness, because it is the one that shrinks at this scale. Solution left in the vial shoulder and the needle hub after the last draw is material you paid for and cannot use. Take 0.05 mL as unrecoverable: on a 10 mL made-up vial that is 0.5%, or 5 mg, worth $0.26, and the effective rate moves from $5.20 to $5.23 per 100 mg. On a 2 mL peptide vial the same 0.05 mL is 2.5%. The gram is the forgiving format here.

Is NAD+ covered by insurance?expand_more

No. Coverage runs through an approved product with a marketing authorisation and a national drug code, and no NAD+ injectable holds either. Research material is sold against an order rather than dispensed against a prescription, so there is no claim for a pharmacy to submit. Clinic infusions are cash services for the same reason.
