Ozempic Dosing Chart: Starting Dose, Schedule and Pen Strengths

Ozempic dosing follows one fixed pattern on the FDA label: start at 0.25 mg once weekly for 4 weeks, then move to 0.5 mg once weekly. From there the dose only goes up, to 1 mg and then 2 mg, if more blood-sugar control is needed, with at least 4 weeks at each dose before the next. Maintenance is 0.5 mg, 1 mg or 2 mg once weekly, and 2 mg is the ceiling.
Each Ozempic pen is built for specific doses, so moving up a step usually means a new pen rather than turning the dial further. Below are the dosage chart week by week, every pen strength and what it can deliver, the schedule rules (day, time, changing the day), the 5-day missed-dose rule, and why counting pen clicks or converting to syringe units is where dosing errors start. Everything is taken from the Ozempic prescribing information on DailyMed (version effective June 2026) and is reference information; the dose itself is set by the prescriber.
Ozempic dosage chart: the escalation week by week
The label sets minimum intervals, not a timetable everyone must follow. The chart shows the earliest each dose can be reached; many people stay at 0.5 mg or 1 mg for good.

One indication has its own target. For adults with type 2 diabetes and chronic kidney disease, where Ozempic is used to reduce the risk of kidney decline and cardiovascular death, the label says to increase to the maintenance dose of 1 mg once weekly after at least 4 weeks on 0.5 mg.

Ozempic starting dose: why it is 0.25 mg
The starting dose of Ozempic is 0.25 mg once weekly for 4 weeks, and the label states the purpose plainly: follow the escalation to reduce the risk of gastrointestinal adverse reactions. The 0.25 mg dose sits under the heading "Recommended Initiation Dosage", and the label's list of maintenance doses begins at 0.5 mg, so 0.25 mg is a lead-in rather than a dose to stay on.
Two numbers from the label explain the 4-week blocks:
- Steady state takes 4 to 5 weeks. With a half-life of about one week, each new dose takes roughly a month to settle, so a step every 4 weeks lets the body adjust before the next increase.
- Nausea rises with dose. In the label's placebo-controlled diabetes trials, nausea was reported by 15.8% on 0.5 mg and 20.3% on 1 mg, against 6.1% on placebo. The full incidence tables are in the semaglutide side effects guide.
After the first step, the label's wording is "at least 4 weeks" before each increase, which is a floor rather than a deadline. Whether to stay longer at a dose, or to stay at 0.5 mg indefinitely, is a prescriber's decision.
Ozempic pen strengths and doses per pen
Ozempic comes in two device types. The multi-dose pens are dial-a-dose but each one only allows certain doses; the single-dose prefilled syringes each hold one fixed dose.

The first pen is the one that carries a person through the start: four 0.25 mg doses for weeks 1 to 4, then two 0.5 mg doses. A month on a pen only works out if the weekly dose matches what the pen was built to deliver. When a pen holds less than a full dose, its dose counter stops short of that dose, and the instructions say not to use that pen for the dose but to start a new one.
Each pen is for one person only and must never be shared, even with a new needle. With a new pen, the instructions call for a flow check (a drop at the needle tip) before the first dose.
Ozempic dosing schedule: day, time and site
The label's administration rules are short:
- Once weekly, on the same day each week, at any time of day, with or without meals.
- Inject under the skin of the abdomen, thigh or upper arm, using a different spot each week when injecting in the same area.
- The weekly day can be changed if needed, as long as the two doses are at least 2 days (more than 48 hours) apart.
- With insulin, give separate injections and never mix the two. Same body region is acceptable, but not right next to each other.
- Check the solution first: it should be clear and colorless, and not used if it contains particles or is discolored.
The 48-hour rule is the one that matters for schedule changes. Moving from a Sunday to a Wednesday injection is fine (3 days after the last dose); pulling the dose forward to the next day is not.
Missed an Ozempic dose: the 5-day rule
Ozempic's missed-dose rule turns on one number:

In both cases the regular once-weekly schedule then resumes. The label gives no instruction to double up, and it does not set out a specific restart plan after several missed weeks; that is a question for the prescriber, who may restart lower on the ladder to limit nausea.
This is a different rule from Wegovy's, which is framed around the next dose (take it if the next dose is more than 2 days away) and does include a re-escalation instruction after 2 or more missed doses. The two are compared on the Wegovy dosing schedule page.
Pen clicks vs compounded units
Ozempic pen clicks
Ozempic pens click as the dose selector turns, and it is tempting to count clicks to set a dose between the printed ones. The Instructions for Use rule that out explicitly: "Do not set the dose by counting the number of clicks you hear." The selector clicks differently forwards and backwards, and only the dose counter and dose pointer show how many milligrams are selected. The label provides no click-to-milligram conversion, so any chart that offers one is not from the manufacturer.
Compounded vials and syringe units
Compounded semaglutide has not been through FDA premarket review and is not equivalent to an Ozempic pen. It usually comes in a multiple-dose vial and is drawn with a U-100 insulin syringe, where 100 units equals 1 mL. The conversion is units = dose in mg ÷ (vial mg ÷ mL of liquid) × 100. The table uses an EXAMPLE concentration of 2.5 mg/mL (for instance 5 mg in 2 mL); a vial at any other strength gives different numbers.

Units are a volume, not a dose. FDA's alert on compounded semaglutide describes patients told to draw 5 units who drew 50, and prescribers who wrote 25 units for an intended 0.25 mg, with patients receiving five to 20 times the intended dose. Ozempic dosing for weight loss
Ozempic's label lists three uses, all in adults with type 2 diabetes: improving glycemic control, reducing major cardiovascular events in those with established cardiovascular disease, and reducing kidney decline and cardiovascular death in those with chronic kidney disease. Weight loss is not one of them, so there is no separate Ozempic dosage for weight loss; the ladder above is the only one the label has.
Weight does fall on Ozempic, and the label reports it. In the trial comparing the top two doses in type 2 diabetes, mean weight change over 40 weeks was -5.6 kg on 1 mg and -6.4 kg on 2 mg, a difference the label says was not statistically significant.
The semaglutide product approved for chronic weight management is Wegovy, which uses a different ladder (adding a 1.7 mg step), a recommended 2.4 mg maintenance dose and, since March 2026, an optional 7.2 mg maximum for adults. Switching from Ozempic injection to tablets
The current oral semaglutide label (Rybelsus and Ozempic tablets, January 2026) includes a switching rule from the injection, and it only covers one starting dose:
- Only patients on 0.5 mg of Ozempic injection are covered by the switch instruction.
- One week after the last 0.5 mg injection, start Rybelsus 7 mg or 14 mg once daily, or Ozempic tablets 4 mg or 9 mg once daily.
- Tablets are taken on an empty stomach in the morning with up to 4 ounces of water, swallowed whole, waiting at least 30 minutes before food, drink or other oral medicines.
Rybelsus and Ozempic tablets are not substitutable milligram for milligram: Rybelsus 7 mg corresponds to Ozempic tablets 4 mg, and 14 mg to 9 mg. The tablet ladders and ceilings are listed on the Ozempic max dose page.

Ozempic has one dosing ladder because it is approved only for adults with type 2 diabetes. The weight-management version of semaglutide is Wegovy, which has its own schedule with 1.7 mg, 2.4 mg and 7.2 mg steps.
