Wegovy Dosing Schedule and Chart: 0.25 mg to Maintenance
Wegovy dosing per the June 2026 label: 16-week ladder from 0.25 mg, maintenance at 1.7, 2.4 or 7.2 mg, pen doses, tablets and the 48-hour missed-dose rule.

Wegovy dosing starts at 0.25 mg injected once weekly and steps up every 4 weeks, through 0.5 mg, 1 mg and 1.7 mg, to reach a maintenance dose from week 17. For adults using it for weight reduction, the label's recommended maintenance dose is 2.4 mg once weekly; 1.7 mg is an approved alternative, and since March 2026 adults who have tolerated 2.4 mg for at least 4 weeks may go up to a 7.2 mg maximum if more weight reduction is clinically indicated.
That is the injection. Wegovy is now also a once-daily tablet with its own 30-day ladder to 25 mg, and the label has rules for switching between the two. Below are the Wegovy dosing chart, the maintenance dose for each approved use, every pen and syringe strength, the tablet schedule, the missed-dose rules and how all of it differs from Ozempic. Figures come from the Wegovy prescribing information on DailyMed (version effective June 2026). It is reference information; the dose and any change to it are set by the prescriber.
Wegovy dosing chart: the 16-week escalation
Unlike Ozempic, where the later steps are optional, the Wegovy label schedules the same four steps for every patient, on a 4-week rhythm, before maintenance. The label's Table 1 applies to all approved uses, in adults and in patients aged 12 and older.

The label builds in one adjustment: if a dose is not tolerated during escalation, consider delaying the next step by 4 weeks. The schedule is the shortest route to maintenance, not a deadline. Wegovy maintenance dose by indication
The Wegovy maintenance dose depends on what it is prescribed for. The label lists four uses for the injection, each with its own options:

For every use, the label says to weigh treatment response and tolerability when choosing between the options. 1.7 mg is not a failed dose: it is an approved maintenance dose in its own right, and the label's trial data show it working (see the results section below).
Wegovy injection doses and pen types
Wegovy injection comes in three device types, and the label tells prescribers to train patients on the specific one they receive and retrain if it changes.

Notice the volumes. The 1.7 mg, 2.4 mg and 7.2 mg single-dose pens all hold 0.75 mL, so their concentrations differ (about 2.27, 3.2 and 9.6 mg/mL). On Wegovy a volume tells you nothing about the dose; only the milligram strength on the pen does. The FlexTouch instructions also warn not to set the dose by counting clicks, and the label says FlexTouch is not recommended for self-injection by people with visual impairment.
Compounded semaglutide in vials is a different product, not reviewed by FDA and not equivalent to these devices. If a prescriber has written a dose for one, the volume is units = dose in mg ÷ (vial mg ÷ mL) × 100 on a U-100 syringe. At an EXAMPLE concentration of 2.5 mg/mL only, 1.7 mg is 0.68 mL (68 units) and 2.4 mg is 0.96 mL (96 units). Wegovy tablets: the daily dosing schedule
Wegovy tablets (first added to the label in December 2025) are approved for weight reduction and cardiovascular risk reduction in adults, not for adolescents or MASH. The ladder runs in 30-day blocks:

The administration rules are strict because absorption depends on them: one tablet each morning on an empty stomach, swallowed whole with up to 4 ounces of water and no other liquid, then at least 30 minutes before food, drinks or other oral medicines. Never more than one tablet a day. If escalation is not tolerated, the label says to consider delaying it; if 25 mg is not tolerated, to consider switching to Wegovy injection 1.7 mg.
Switching between Wegovy injection and tablets
The label gives exact timing in both directions:
- Injection to tablets: patients on Wegovy 2.4 mg injection can switch. Start 25 mg tablets once daily one week after the last 2.4 mg injection.
- Tablets to injection: the day after the last 25 mg tablet, start Wegovy 2.4 mg once weekly. For patients who did not tolerate 25 mg tablets, consider 1.7 mg injection instead.
- Type 2 diabetes on 25 mg tablets needing more weight reduction: consider switching to 1.7 mg injection and then following the injection escalation.
Tablet and injection milligrams are not interchangeable. 25 mg by mouth daily and 2.4 mg by injection weekly are paired on the label because of absorption, not because the numbers are equivalent.
Missed Wegovy doses
Wegovy's injection rule is framed around the next dose, not the missed one:

The re-escalation row is the one people miss. After two or more skipped weeks, going straight back to 2.4 mg risks the nausea and vomiting the ladder exists to avoid; which lower dose to restart at is for the prescriber. The Medication Guide also allows changing the injection day, as long as the last dose was given 2 or more days before.
How Wegovy dosing differs from Ozempic

What the maintenance dose choice changes
The label's own trials put numbers on the 1.7 mg versus 2.4 mg choice. Populations and durations differ between studies, so compare within a row, not across rows.

If the next scheduled dose is more than 2 days away, inject as soon as possible; if it is less than 2 days away, skip and resume on the regular day. If 2 or more doses in a row are missed, the label says to restart escalation at a lower dose.
Is 7.2 mg now the standard Wegovy dose?expand_more
No. 2.4 mg remains the recommended maintenance dose. 7.2 mg, approved by FDA on March 19, 2026, is an optional maximum for adults using Wegovy for weight reduction who have tolerated 2.4 mg for at least 4 weeks and need more weight reduction.
Are Wegovy and Ozempic doses interchangeable?expand_more
No. They contain the same molecule but have different approved uses, devices, maintenance doses and missed-dose rules, and the Wegovy label advises against using it with other semaglutide products. Switching between them is a prescriber's decision.
