Maximum Dose of Ozempic for Weight Loss: The 2 mg Ceiling
Ozempic's maximum dose is 2 mg weekly, on a diabetes label; Wegovy now goes to 7.2 mg. Every semaglutide ceiling from current FDA labels, and why it exists.

The maximum dose of Ozempic for weight loss question has a two-part answer. Ozempic's prescribing information caps the injection at 2 mg once weekly, and that label is written for type 2 diabetes, not weight loss. The semaglutide injection approved for weight reduction is Wegovy, and its label now allows adults to go up to 7.2 mg once weekly (approved by FDA on March 19, 2026), with 2.4 mg still the recommended maintenance dose.
So a semaglutide max dose depends entirely on which product is in the box: two injections and three tablets share the same molecule but carry five different ceilings. This page lists each one exactly as the current FDA labels print it, explains why a ceiling exists at all, what the labels say about going above it, and what a maximum dose works out to in syringe units for anyone handed a compounded vial instead of a pen. It is label reference information, not a recommendation to use any particular dose; dose changes are a prescriber's decision.
Ozempic max dose: 2 mg once weekly
The Ozempic label (DailyMed version effective June 2026) is direct: the maximum recommended dosage is 2 mg once weekly. The route to it is fixed. Everyone starts at 0.25 mg for 4 weeks, moves to 0.5 mg, and the label then allows two further steps only if additional glycemic control is needed:
- 0.5 mg to 1 mg, after at least 4 weeks on 0.5 mg
- 1 mg to 2 mg, after at least 4 weeks on 1 mg
Counting those minimum intervals, week 13 is the earliest anyone could reach 2 mg. Plenty of people never do, because 0.5 mg and 1 mg are equally valid maintenance doses on the label. For the chronic kidney disease indication the label names 1 mg as the maintenance dose rather than 2 mg.
The 2 mg dose has its own pen: an 8 mg/3 mL pen (2.68 mg/mL) that can only be set to 2 mg and holds four doses. There is no Ozempic pen that dials above 2 mg. The full week-by-week ladder and the pen for each step are on the Ozempic dosage chart.
Semaglutide max dose by product
Searches for "maximum dose of semaglutide" usually mean one of these five products. Their ceilings differ because their routes, indications and trial programmes differ.

Two things in that table trip people up. First, oral and injected milligrams are not comparable: semaglutide is poorly absorbed from the gut, so tablet doses are far larger numbers for a similar effect. Second, even the tablets are not interchangeable with each other. The combined Rybelsus and Ozempic tablets label states the two are not substitutable on a mg-to-mg basis; its own conversion pairs Rybelsus 7 mg with Ozempic tablets 4 mg, and Rybelsus 14 mg with Ozempic tablets 9 mg.
The highest dose of semaglutide on any US label is therefore 7.2 mg per week by injection, and that applies only to adults using Wegovy for weight reduction. The Wegovy label states that the safety of the 7.2 mg injection has not been established in pediatric patients or in patients with MASH.
Why the ceiling exists
A label maximum is not a toxicity threshold. It is the highest dose the manufacturer studied, submitted and had approved with an acceptable balance of benefit and adverse reactions. Several things in the Ozempic label show how that balance is struck.
Exposure rises in step with dose
Semaglutide exposure increases dose-proportionally across 0.5, 1 and 2 mg. In the trial that compared the two top doses, mean steady-state concentrations were 111.1 ng/mL on 1 mg and 222.1 ng/mL on 2 mg. Doubling the dose doubles what is in the blood; there is no plateau that makes extra milligrams harmless.
Side effects climb with it
In that same 40-week trial, gastrointestinal adverse reactions occurred in 34% of patients on 2 mg against 30.8% on 1 mg. The escalation ladder exists for the same reason: the label says to follow it to reduce the risk of gastrointestinal adverse reactions.
The extra benefit gets smaller
Weight is the clearest example. In the 1 mg versus 2 mg trial, mean body weight fell 5.6 kg on 1 mg and 6.4 kg on 2 mg over 40 weeks, and the label states the difference was not statistically significant. That is the honest answer to "what is the maximum dose of Ozempic for weight loss": the 2 mg step was approved for blood glucose, not for extra weight reduction.
The half-life makes mistakes slow to undo
Semaglutide's half-life is about one week and steady state takes 4 to 5 weeks of weekly dosing. A dose above the ceiling cannot be cleared quickly, which is why the overdose section of both injection labels warns that a prolonged period of observation may be needed.
Wegovy 7.2 mg: the higher ceiling, and who it applies to
Our semaglutide side effects guide discusses a 7.2 mg dose, which can look inconsistent with a 2 mg Ozempic maximum. Both are correct; they belong to different labels. FDA approved the 7.2 mg weekly Wegovy dose for weight reduction and long-term maintenance on March 19, 2026 (supplement NDA 215256/S-029), and the current Wegovy label states:
- The recommended adult maintenance dose for weight reduction remains 2.4 mg, with 1.7 mg as an alternative.
- For patients who tolerate 2.4 mg for at least 4 weeks and when additional weight reduction is clinically indicated, the dose may be increased to a maximum of 7.2 mg once weekly.
- The 7.2 mg dose comes only as a single-dose pen branded Wegovy HD, holding 7.2 mg in 0.75 mL.
- For cardiovascular risk reduction, adolescents and MASH, the maintenance options stay at 2.4 mg or 1.7 mg.
The label's trial data show what the higher ceiling buys and costs. In the 72-week study in adults with obesity without diabetes (Study 8), mean weight change was -18.8% on 7.2 mg, -15.5% on 2.4 mg and -3.9% on placebo, a difference of 3.3 percentage points between the two active doses. Average steady-state concentration was about 230 nmol/L on 7.2 mg versus about 75 nmol/L on 2.4 mg. Dysesthesia (altered skin sensation) was reported by 22% on 7.2 mg, 6% on 2.4 mg and 0.3% on placebo, and hair loss by 5.8% versus 3.3% versus 1.0%.
None of this changes Ozempic. The Ozempic label still tops out at 2 mg, and there is no Ozempic pen above 2 mg. How Wegovy reaches its maintenance dose step by step is covered in the Wegovy dosing schedule.
What the labels say about going above the maximum
The labels never describe a dose above their stated maximum, and several instructions exist specifically to prevent stacking doses:
- One product at a time. The Wegovy label's limitation of use says it should not be used with other semaglutide-containing products or any other GLP-1 receptor agonist. Taking Ozempic and Wegovy together, or adding a compounded product on top of either, is outside the label.
- One tablet a day. The Rybelsus, Ozempic tablets and Wegovy tablets labels all state: do not take more than one tablet per day.
- No doubling after a missed dose. Ozempic's rule is to take a missed dose within 5 days or skip it; Wegovy's is to skip it if the next dose is less than 2 days away. Neither label allows a double dose to catch up.
- Doses at least 2 days apart. Ozempic allows the weekly day to move only if the two doses are at least 2 days (more than 48 hours) apart.
The overdose section describes what happens when those rules break down. The Wegovy label notes that overdoses with GLP-1 receptor agonists have caused severe nausea, severe vomiting and severe hypoglycemia, advises supportive treatment and contacting Poison Help (1-800-222-1222) or a medical toxicologist, and warns that the roughly one-week half-life may require a prolonged period of observation. There is no antidote listed.
If a current dose is not producing the expected effect, the label pathway is the next approved step or a different approved product chosen by a prescriber, not an off-label amount from the same pen.
Highest dose vs best dose: what the trial numbers show
Every step up the semaglutide ladder has been tested head to head against a lower one, and the labels publish the results. Read together, they show each extra milligram adding less than the one before:

The populations, durations and endpoints differ between rows, so the rows should not be compared with each other, only within themselves. Within each, the pattern is the same: the higher dose adds something, and the side-effect rate rises alongside it. That trade-off is why every label treats its top dose as optional rather than as a target.
Units at the max dose for compounded vials
Branded Ozempic and Wegovy are dosed in milligrams by a pen or single-dose device. Compounded semaglutide is typically supplied in multiple-dose vials at concentrations that vary by compounder, and is drawn up in a U-100 insulin syringe marked in units. Compounded products have not been through FDA premarket review and are not equivalent to the branded pens; nothing below implies that a compounded product has an approved maximum.
The arithmetic is: units = dose in mg ÷ (vial mg ÷ mL of liquid) × 100 on a U-100 syringe, where 100 units = 1 mL. The table uses two EXAMPLE concentrations only: 2.5 mg/mL (for instance 5 mg in 2 mL) and 5 mg/mL. Your vial's label is the only concentration that counts.

The last row is the point. A 1 mL U-100 syringe holds 100 units, so at either example concentration a 7.2 mg volume does not fit in one syringe. The branded Wegovy HD pen delivers 7.2 mg in 0.75 mL, a concentration of 9.6 mg/mL, which is why the dose is practical in that device and not a number to reproduce from a vial.
FDA has documented what goes wrong with units. Its alert on compounded semaglutide describes patients drawing 50 units instead of 5 and receiving five to 20 times the intended dose, and prescribers converting milligrams to units incorrectly, for example writing 25 units when 0.25 mg (5 units at that product's concentration) was intended.
