Saxenda Dose: Starting Dosage, Weekly Escalation and Missed Dose
Saxenda dose from the FDA label: 0.6 mg daily in week 1, up 0.6 mg each week to 3 mg from week 5. Missed-dose rule, teen dosing, pen volumes and when to stop.

The Saxenda dose for weight management is 3 mg injected once daily, reached by a weekly climb: 0.6 mg a day in week 1, then 1.2 mg, 1.8 mg and 2.4 mg, and 3 mg from week 5 onward. For adults the lower doses are for titration only, and the label says to stop Saxenda if 3 mg cannot be tolerated or if less than 4% of starting body weight has been lost 16 weeks after starting. Adolescents aged 12 and older with obesity and a body weight above 60 kg follow the same schedule, with an option to stay at 2.4 mg.
Every figure below is taken from the current US prescribing information for Saxenda (liraglutide), revised 02/2026, with Instructions for Use revised 06/2026. It is reference information; the prescriber sets the actual schedule.
Saxenda starting dosage and the five-week escalation
The Saxenda starting dosage is 0.6 mg once daily for one week. After that the label's Table 1 adds 0.6 mg each week until 3 mg is reached. Each pen holds 6 mg/mL, so every 0.6 mg step is another 0.1 mL.

The first four weeks use 42 mg in total, a little over two 18 mg pens. Saxenda is injected once a day at any time, with or without meals, in the abdomen, thigh or upper arm; the label adds that injection sites should be rotated within the same region. When escalation is slowed down
The weekly timetable is the default, not a requirement to push through symptoms. The label allows for slowing it:
- Adults: if an increased dose is not tolerated during escalation, consider delaying the next increase by about one extra week.
- Adolescents: if an increased dose is not tolerated, it may be lowered to the previous level, and escalation may take up to 8 weeks instead of 4.
What the label does not offer adults is a permanent stop below 3 mg. For adults the recommended dose is 3 mg and the lower doses are "for titration only"; if 3 mg cannot be tolerated, the label says to discontinue rather than settle at 2.4 mg. That is a real difference from the weekly drugs, where lower maintenance doses are part of the label. Saxenda missed dose: what the label says
Because Saxenda is daily, a Saxenda missed dose is a small event as long as the gap stays short. The prescribing information and the patient Medication Guide say the following.

The restart rule is the part that catches people out. Tolerance to nausea fades quickly, so going straight back to 3 mg after a long break is exactly the jump the four-week ladder was designed to avoid. If you have missed several days, the restart plan is a conversation with the prescriber.
Saxenda dosing for adolescents aged 12 to 17
Saxenda is labelled for pediatric patients aged 12 and older who have obesity and a body weight above 60 kg. The dose ladder is the same, but three rules differ from the adult label:
- Maintenance: 3 mg daily is recommended, but adolescents who do not tolerate it may have the maintenance dose reduced to 2.4 mg daily.
- Stopping for intolerance: discontinue if 2.4 mg cannot be tolerated.
- Response check: evaluate BMI after 12 weeks on the maintenance dose and discontinue if BMI has not fallen by at least 1% from baseline.
The label's 56-week adolescent trial (251 pubertal patients aged 12 to 17) titrated to 3 mg over 4 to 8 weeks; 82.4% reached 3 mg and for 8.8% the maximum tolerated dose was 2.4 mg. The label also notes that hypoglycemia occurred in adolescents on Saxenda even though none had type 2 diabetes. Saxenda's safety and effectiveness have not been established in pediatric patients with type 2 diabetes.
When to stop Saxenda per the label
The Saxenda label is unusual in writing a response test into the dosing section. The stop points are:
- Adults, no response: evaluate body weight 16 weeks after starting; discontinue if at least 4% of baseline weight has not been lost, because meaningful, sustained loss is unlikely with continued treatment.
- Adults, intolerance: discontinue if 3 mg cannot be tolerated.
- Adolescents: less than 1% BMI reduction after 12 weeks on maintenance, or intolerance of 2.4 mg.
- Safety stops: suspected pancreatitis, a sustained increase in resting heart rate, or a serious hypersensitivity reaction. When a pregnancy is recognised, the label says to discontinue.
Note that the 16-week check includes the four escalation weeks, so it falls after roughly 12 weeks at 3 mg. Two recent label changes are worth knowing if you read an older copy: pregnancy was removed from the contraindications in 05/2025 (the pregnancy section still advises stopping once pregnancy is recognised), and the warning on suicidal behaviour and ideation was removed in 02/2026.
Saxenda injection dose in the pen, and compounded liraglutide
The Saxenda injection dose is dialled, not drawn. Each prefilled pen holds 18 mg in 3 mL (6 mg/mL) and the dose selector offers 0.6, 1.2, 1.8, 2.4 and 3 mg. By arithmetic, one pen gives at most six 3 mg doses; each new pen needs a one-time flow check, and a pen in use is discarded 30 days after first use. Cartons hold three or five pens. Unused pens are refrigerated at 2 to 8 °C and never frozen; a pen in use can stay at 15 to 30 °C or in the fridge for those 30 days.
Liraglutide from a vial drawn with an insulin syringe is not Saxenda and is not interchangeable with it. If a prescriber has supplied one, the unit count depends entirely on concentration:
Units on a U-100 syringe = dose in mg ÷ (vial mg ÷ mL of water) × 100
EXAMPLE concentration only: 10 mg mixed with 2 mL gives 5 mg/mL, so 0.6 mg is 12 units, 1.2 mg is 24, 1.8 mg is 36, 2.4 mg is 48 and 3 mg is 60 units. A different concentration changes every figure. What the 3 mg dose achieved in the label trials
All three adult 56-week trials titrated to 3 mg over four weeks alongside a reduced-calorie diet and activity counselling. The label reports these mean results:
